Please have your pharmacy fax refill requests to the office allowing for a 48hr turn around time. If requiring medication renewals prior to your initial intake appointment please call our office.

How do I enroll?

To begin the enrollment process, we will require your name, date of birth, contact information and OHIP information. Please schedule your initial intake appointment, focusing on a comprehensive medical history review.

Can I book an urgent care need appointment prior to my initial intake appointment?

If you require urgent same-day care before your scheduled intake appointment, please call our office, and we will do our best to accommodate your needs promptly.

How are follow-up and regular appointments booked?

To book a follow-up appointment for any of your health needs our phone lines are open during office hours and messages can be left after hours.
